HOW
DO I... OBTAIN
TELEPHONE ADVICE?
If you wish to speak to your doctor,
please call at the times indicated below, which the doctors generally try to keep
clear for phone calls:
Surgeries are always by appointment. Ideally each patient should see their own doctor for medical care. If you do not need an appointment within two working days, you also have the option to book up to six to eight weeks in advance, if this is more convenient to you. Please phone in an afternoon when the lines are less busy. More urgent problems will be seen within 24 hours but not necessarily by your own doctor.
Please note that three year reviews and annual reviews (for the over 75s) are available
on request. Please see reception if you wish to see your medical records. There
may be a fee payable.
HOW DO I... OBTAIN A HOME VISIT?
Requests for home visits should
be made before 11.00am. The receptionist may need to ask for some details of the
problem. Please consider the necessity for a home visit; a child with a high temperature
can safely be brought to the surgery where a more complete assessment is possible.
Home visits are intended for the housebound and those too ill to travel. Requests
for urgent medical attention which may involve a home visit are referred to the
duty doctor.
HOW DO I... OBTAIN TEST RESULTS?
The results of tests and investigations
are usually discussed with the doctor at a follow-up appointment. However, if
your doctor has asked you to telephone for a result please telephone reception
after 2.00pm, allowing three days for blood tests and 10 days for x-ray results.
Please note that patient information is only given to the patient (normally aged
over 16) and not to a relative, unless consent has previously been given.
HOW
DO I... CHANGE
MY DETAILS?
Please advise reception
if you change your name or address. It would be a great help if you
would also ensure that we have your telephone number.
HOW
DO I... MAKE
A COMPLAINT?
We operate a practice-based
complaints procedure as part of the NHS system for dealing with complaints. Our
system meets the national criteria. Our aim is to give you the highest possible
standard of service and we try to deal swiftly with any problems that may occur.
Our Practice Administrator, Mrs Yvonne Gray, can give you further information. Any comments on our provision
of services, or compliments, are also welcomed.
HOW DO I... OBTAIN A REPEAT PRESCRIPTION?
Treatment cards for routine medications
are issued by your doctor and can be used to order further prescriptions. Please
allow two working days for the request to be processed (enclose a stamped, addressed
envelope if you wish us to post it). If you require a medication which is not
listed on a treatment card, please make an appointment with your doctor to discuss
it. Patients can arrange for a pharmacy to pick up their prescription directly
from the surgery. Please contact reception for details.
A repeat dispensing or ‘ batch system’ is available for patients on repeat prescriptions who use the same pharmacy each time. A batch of prescriptions is issued and then lodged with your chosen pharmacy. The system has the advantage that you can have a prescription dispensed early if you are going away. Patients generally need to see their doctor for a check-up before the next batch is issued .
In order to avoid errors, WE DO NOT ACCEPT REQUESTS FOR REPEAT PRESCRIPTIONS BY
TELEPHONE.
The following pharmacies offer a prescription collection and delivery service;
Please contact our receptionist who will inform you how
to register. All new patients will need to book a new patient check appointment. Please tell our receptionist if
you are a carer or if you have a carer.
Patients have the right to express a preference of practitioner. The
practice will endeavour to comply, although it might not always be possible, in which case an explanation will be
given.
