Join the practice

To register as a permanent patient please call at Reception to collect a New Patient Registration Pack, this contains relevant forms which will need to be completed for each person. The receptionist will need to see photo ID (driving licence/Passport) for any adults registering.

All adults registering will need to book a new patient check appointment with the Healthcare Assistant. Please tell our receptionist if you are a carer or if you have a carer.

Patients have the right to express a preference of practitioner. The practice will endeavour to comply, although it might not always be possible, in which case an explanation will be given.

Everyone is welcome in general practice.  You do not need proof of ID, address, immigration status or an NHS number in order to receive care or see a GP

The Practice does offer patients access to Online Services which allows the patient to book appointments directly with their usual GP and to order repeat medication online. This can be applied for once patients registration is complete.


Live within boundary area?

To be eligible to register at the practice, you must live within the practice catchment area.
